Patient Info

Welcome to the Pain Center of Western Washington! Our Providers and the staff are looking forward to having the opportunity to provide you with some definitive answers and treatment options to relieve your pain. Here is some helpful information for new patients before your first visit.

NEW PATIENTS

  • PCWW requires a referral from your PCP or specialist.
  • PCWW does not provide chronic medication management, either of opiate or non-opiate medication.
  • Initial appointments are for consultation, evaluation and recommendations only. No treatment or medications will be provided on your first visit Any recommended treatment will take place on subsequent visits
  • PCWW is currently contracted and a participating provider with most insurance carriers. It is your responsibility to verify if your insurance is contracted. We will do our best to verify coverage and eligibility, but since your health insurance policy is a contract between you and that company, ultimately you are responsible for knowing and understanding your insurance coverage.
  • A copy of your insurance card is required at each visit
  • Payment is expected at the time of visit. This includes co-payments, coinsurance, deductibles or any amount not covered by insurance.
  • If you cannot keep your scheduled appointment, please contact our office at least 24 hours in advance. Failure to do so may result in a NO – SHOW charge to your account, $75 for office visits and $100 for procedures.
  • Please complete the new patient forms prior to your appointment. Due to the amount of time these forms take to complete, your appointment may be rescheduled if they are not completed.

Insurance

The Pain Center of Western Washington is currently contracted and is a participating provider with most insurance carriers.

Due to the differences in plans you should check with your insurance company for the details of your plan, as we are not contracted with some of the HMO style plans, in most cases a referral will satisfy these plans.

If you do not see your carrier please feel free to contact our office or your carrier directly.
